  • Payments: We accept cash, checks, and credit cards. There will be a 15.00 fee for any returned checks. If more than two checks are returned, we will require payments be made with money order or bank check.
  • Medicare: Dr, Naidu is a participating medicare provider. Patients are required to pay medicare co-payments, deductibles, and all non-covered services. We will bill secondary insurances for claims not covered by medicare; however, the patient is still responsible for all claims and services not covered by either insurance.
  • HMO and PPO plans: We will accept assignment on all HMO and PPO insurance plans on which the provider is a participating provider. Patients are responsible for all deductibles, co-payments and non-covered services.
  • Copayments: Copayments are expected and must be made at the time of service. A $10.00 charge will occur if we have to bill you for your copayment. Please also note that when medical problems are presented the same day as the yearly physical, and the physician orders medications, tests, referrals and etc. that are not part of your yearly preventative care; the office is entitled to bill the patients insurance company for an additional fee for time spent managing these problems. If this occurs a copayment will  be due depending on your insurance company's policy.
  • NO insurance payment expected: no insurance is expected at the time of your service. Our office will be happy to assist you in setting up a payment plan if you are unable to pay the visit in full at the time of service. A minimum of 5.00 a month is expected unless a financial hardship is made known to the provider prior to services being rendered. If you pay in full at the time of your services a 15% discount will be given.
  • Missed appointments: Patients who fail to show for their appointments, and do not call us ahead of time to cancel will be subject to a $25.00 fee. This charge will be your resposibility and cannot be charged to your insurance carrier and must be paid in full before your next appointment. Some exceptions will be made in case of an emergency. 

    Patients who have 3 or more missed appointments (no call and no show), may be terminated from the practice.
